چکیده

INTRODUCTION: Pedicle screw (PS) constructs allow for better curve correction in the coronal plane as well as in the axial plane in patients with adolescent idiopathic scoliosis (AIS)[1, 2]. However, reduction of physiological kyphosis of the thoracic spine has been one of the problems associated with the PS constructs [3]. The hypothesis for the hypokyphogenesis is that the direct vertebral derotation obtained by PS can render the thoracic spine more straight in the sagittal plane. The purpose of this study was to prove this hypothesis by computer simulated corrections using a three-dimensional (3D) scoliosis model.

Vertebral derotation in adolescent idiopathic scoliosis causes hypokyphosis of the thoracic spine

BACKGROUND The purpose of this study was to test the hypothesis that direct vertebral derotation by pedicle screws (PS) causes hypokyphosis of the thoracic spine in adolescent idiopathic scoliosis (AIS) patients, using computer simulation. METHODS Twenty AIS patients with Lenke type 1 or 2 who underwent posterior correction surgeries using PS were included in this study. Simulated corrections...

Interdependency between corrections in the three anatomic planes in AIS instrumentation

Methods A validated patient-specific biomechanical modeling and simulation technique was used to assess the 3D correction of a Lenke-1 AIS case through posterior spinal instrumentation. Uniplanar pedicle screws were bilaterally placed at the 2 proximal, 2 distal, and 3 apical levels. The simulation steps included only the concave side 5.5 mm CobaltChrome rod attachment and its derotation follow...

Bilateral apical vertebral derotation technique by vertebral column manipulation compared with vertebral coplanar alignment technique in the correction of lenke type 1 idiopathic scoliosis

BACKGROUND Widely used rod rotation and translation techniques for idiopathic scoliosis (IS) are effective in correcting spinal coronal deformity. Bilateral apical vertebral derotation technique by vertebral column manipulation (VCM) and vertebral coplanar alignment (VCA) technique are two strategies for three-dimensional (3D) correction for IS. The purpose of this study is to compare the post-...

Is there a better derotation manoeuvre in posterior correction of thoracic adolescent idiopathic scoliosis?

Background Retrospective review of 62 consecutive patients affected by AIS (Lenke type 1 or 2) treated by posterior fusion with pedicle screw-only instrumentation. Three groups identified: Pre-Rod (direct derotation procedure done before inserting rods), Single-Rod (derotation done after concave rod insertion) Double-Rod (after both rods). The Pre-Rods insertion cases showed a significantly bet...

Pedicle Screw Instrumentation for Adolescent Idiopathic Scoliosis: The Insertion Technique, the Fusion Levels and Direct Vertebral Rotation

The pedicle is a power nucleus of the vertebra and offers a secure grip of all 3 columns. Pedicle screw instrumentation has advantages of rigid fixation with improved three-dimensional (3D) correction and it is accepted as a reliable method with a high margin of safety. Accurate placement of the pedicle screws is important to reduce possible irreversible complications. Many methods of screw ins...
